  • Position Summary

    Westmed Medical Group is hiring for a full-time Certified Nutritionist at the Purchase, New York office location.  General schedule is Monday to Friday 8am to 5pm with variation to be expected based on patient needs.  Travel to other office sites may be required.  The Certified Nutritionist provides health care under physician supervision.  Provides direct nutrition counseling and education and assists in developing nutrition education activities. Assists in the assessment of Westmed programs and its goals related to nutrition and disease. Participates in all quality assurance activities; collects, and compiles data for the nutrition services as well as Medical Home and ACO initiatives. Participates in staff development activities and acts as a liaison with all nursing areas interacting with the nutrition services. Complies with Westmed departmental policies and procedures and maintains good patient and interdisciplinary working relationships.

    Essential Job Functions & Responsibilities

    • Provides individualized nutrition self-management education/ training to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional performance for Certified Nutritionist.
    • Provides direct nutrition patient education and assists in developing nutrition patient education activities.
    • Responsible for utilizing  professional skills and experience to educate patients with disease management and nutrition issues.
    • Acts as a liaison to WMG PCP’s, Hospitalists, Snifists and other medical staff regarding newly diagnosed patients with nutrition problems as well as chronic/complex patients to provide/coordinate the medically necessary and appropriate nutritional needs and diet restrictions of patient.
    • Counsel individuals and groups on basic rules of good nutrition, healthy eating habits, and nutrition monitoring to improve their quality of life.
    •  Assess nutritional needs, diet restrictions and current health plans to develop and implement dietary-care plans and provide nutritional counseling.
    • Advise patients and their families on nutritional principles, dietary plans and diet modifications, and food selection and preparation.
    • Develops patient care plans in collaboration with patient/caregiver/provider.

     

    For Bariatric patients:

    • Assesses patient nutrition requirements using appropriate physiological data and ASMBS guidelines.
    • Develops nutrition care plans for patients based on medical nutrition therapy orders and/or assessment data, and modifies these plans as necessary during treatment.
    • Counsels patients/family on nutrition intervention plan pre- and post-surgery.
    • Communicates with the physician and other members of the health care team to coordinate medical nutrition intervention with other aspects of medical care.
    • Plans, organizes, instructs, and evaluates nutrition classes and support groups.
    • Prepares patients for bariatric surgery following established protocols.
    • Completes timely, thorough, and professional documentation of all medical nutrition therapy counseling or interventions.
    • Participates in evaluating, selecting, developing, and updating nutrition education materials/handouts and revision of treatment protocols, according to evidence-based medical nutrition therapy research and practice.
    • Acts as a professional resource to assess educational needs, and then plans and teaches classes for providers and staff on nutritional treatment options.
    • Attends and participates in departmental meetings at designated Westmed locations up to two times per month.
    • Provides nutritionist coverage to the assigned Westmed locations as needed.

     

     

    General responsibilities

     

    • Collects DSME/T program participant assessment data, in a collaborative and ongoing manner.
    • Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSME/T program participants.
    • Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.
    • Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.
    • Conducts a follow-up assessment upon completion of DSME/T and/or ASMBS program services, using outcome measures.
    • Documents assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records.
    • Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
    • Identifies when a program participant’s needs are outside the scope of the instructor’s practice and expertise, plus arranges for additional services to meet needs.
    • Acts as a liaison to provide ongoing support and education to the family/caregiver system in order to lessen enhance self -management and lessen disease anxiety and decrease negative outcomes.
    • Solves complex problems and/or conducts moderately complex analyses impacting disease management, Medical Home and ACO diabetes composite and associated Metrics.
    • Develops care plans for complex cases in collaboration with patient/caregiver/provider.
    • Works with minimal guidance, seeks guidance on only the most complex tasks.
    • Translates concepts into practice.
    • Provides explanations and information to others on difficult issues.
    • Coaches, provides feedback, and guides others.
    • Assesses the effectiveness and quality of services provided related to clinical, functional, and financial impact and outcomes.
    • Refers appropriately to and maximizes use of community services, support programs, internal/external resources available to patient including Embedded Case Management, Palliative Medicine, Behavioral Health, Diabetic Education, etc.
    • Educates and supports patients  to assist them in the best decision making for their health and wellness.
    • Act as a resource for others with less experience..
    • Adheres to ethical, legal/regulatory and accreditation standards.
    • Other duties as assigned.

    Required Qualifications

    • B.S. Degree in nutrition or dietetics, Master’s degree is preferred
    • Registration through the American Dietetic Association
    • Certification by the New York State Education Department as Dietitian/Nutritionist
    • Certification by one or more of the following - Commission on Dietetic Registration (CDR) of the Academy of Nutrition and Dietetics (AND), the Certification Board for Nutrition Specialists (CBNS) or the Clinical Nutrition Certification Board (CNCB)
    • Minimum of two years clinical experience
    • Current valid NY and CT professional license
    • Certification as Diabetes Educator preferred
    • Prior experience working with a bariatric surgery practice and bariatric patient population
    • Prior experience working with diabetic patients
    • Insulin pump experience preferred
    • Additional experience in any of the following is desirable: chronic condition management of patients with asthma or hypertension; nutrition teaching, motivational interviewing, weight management
    • Astute triage skills required
    • Transitional coaching experience a plus
    • Knowledge of Medicaid, Medicare, and government/CMS regulations preferred
    • Bilingual Spanish preferred
    • Must have computer skills and technical aptitude, Electronic Medical Record experience Preferred
