Position Summary

Westmed Medical Group has a full time position available for a full-time Infusion RN/Case Manager for the infusion center at the Norwalk, CT office.  The RN will function as both an Infusion RN and Case Manager in a dual function role.  RN will be responsible for infusions and accessing the patient for bloods/chemo via peripheral lines.  RN will be trained to administer chemotherapy, and provide monitoring and continued assessment throughout the infusion period. The RN will also mix chemo.  We are willing to teach access via port-a-cath.  In the case management function, RN will be responsible for assisting the primary care providers in the coordination of care for a high-risk patient population, ensuring optimal service coordination, facilitation and support through all transitions of care.  This may include but not be limited to hospitalizations, sub-acute stays and ER visits.


Schedule is Monday to Friday 8:00am to 5:00pm with flexibility in hours required as needed.

Essential Job Functions & Responsibilities

  • Responsible for infusions and accessing the patient for bloods/chemo via peripheral lines.
  • Administer chemotherapy, and provide monitoring and continued assessment throughout the infusion period.
  • Assesses and monitors changes in the physical and psycho/social needs of patients utilizing the nursing process.
  • Educates and counsels patient and/or caregiver with provisions for follow-up.
  • Prepares the patient for and assists in complex procedures and treatments.
  • Administers and documents all medications including IV therapy accurately.
  • Acts as a liaison between patients, their families, the physician, and other health care personnel to promote the delivery of quality, efficient and cost-effective healthcare services.

  • Communicates effectively to the healthcare provider(s) any changes in the patient’s plan of care to promote care continuity.

  • Initiates post hospitalization or ER visit contact by telephone to facilitate post-hospital care including understanding of the discharge plan, acquisition and understanding of new medications or changes in medications as well as establishing necessary follow-up appointments.

  • Provides regular progress updates to the Primary Care Provider; assists with discharge plan in collaboration with the discharge planning nurse at the transitional site of care.

  • Follows-up on the targeted patient population referrals, internal and external to the practice.

  • Educates patients and families about the chronic illness and promotes a self-care model, when appropriate to achieve optimum wellness or stability of the illness.

  • Reviews patient reports and retrieves the necessary information to follow up on any results or outstanding issues.

Required Qualifications

  • BSN degree
  • Current Connecticut licensure as a Registered Nurse required, dual licensure in New York strongly preferred
  • ONS/ONCC Chemotherapy Biotherapy Certified
  • Preferred Accredited Case Manager Certification (ACM)
  • Current BLS
  • 3-5 years experience as a RN Case Manager in a hospital or medical facility
  • Strong computer skills, EMR experience preferred
  • Excellent communication (written & verbal) and interpersonal skills
  • Strong critical thinking skills
  • Ability to multi-task in a fast paced environment
