  • Position Summary

    Transition of care at Westmed Medical Group is EXPANDING!  Multiple positions are currently available for a full-time Complex Care Nurse Practitioner based on office location.  The Complex Care Nurse Practitioner will be embedded in each office within the Internal Medicine department, collaborating with the physicians and case managers.  

     

    Primary responsibilities include delivering high-touch, high-value assessment and care management services for complex and frail patient in a multispecialty group practice. Practice includes in-office and in-home patient engagement, which allows for comprehensive patient-centered care.  Patient care and assessment in collaboration with the PCP and multispecialty care team including full medical history, medication review, socioeconomic assessment, population health and use of data analytics  to capture a comprehensive picture of the patient's health status and appropriate care planning and motivate change for desired outcomes.

     

    Position also involves oversight and engagement of high risk patients, associated resources and utilization between health care settings and all transitions to facilitate the appropriate level of care based on the severity of illness and intensity of service. Transitions management includes in-office and in-home direct medical care, assessment, planning and intervention, medication reconciliation, referral and patient education on red flag warning signs through patient activation techniques.  Acts as a liaison to post-acute providers and Westmed PCP/care team to effect appropriate utilization metrics.

     

    Note:  This position requires frequent patient interaction in the physician practice setting and within patient homes.

    Essential Job Functions & Responsibilities

    • Assesses, diagnoses and treats the patient in the office and when needed in patient’s home
    • Collaborates and communicates with patient's primary care provider and specialists, hospitalists and SNFists
    • Refers to specialists
    • Collaborates with embedded Case Management Team and Pharmacist
    • Conducts Serious Illness conversations
    • Orders labs and other diagnostic tests
    • Medication reconciliation & adherence
    • Prescription writing, orders, refills
    • Patient & family education and support
    • Assesses and interprets patient needs, gaps, and requirements
    • Acts as a liaison to WMG Hospitalists, SNFists, PCP’s and other medical staff regarding patient transitions in order to provide/coordinate the medically necessary and appropriate care for the patient
    • Acts as a liaison to post-acute providers and Westmed PCP/care team to effect appropriate utilization metrics
    • Acts as a liaison to provide ongoing support and education to the family/caregiver system in order to lessen transition anxiety and decrease negative outcomes
    • Solves complex problems and/ or conducts moderately complex analyses impacting transitions/readmissions
    • Develops care plans for complex cases in collaboration with patient/caregiver/provider
    • Works with minimal guidance, seeks guidance on only the most complex tasks
    • Translates concepts into practice
    • Assesses the effectiveness and quality of services provided related to clinical, functional, and financial impact and outcomes
    • Refers appropriately to and maximizes use of community services, support programs, internal/external resources available to patient including Embedded Case Management, Palliative Medicine, Behavioral Health, Diabetic Education, etc
    • Educates and supports patients to assist them in the best decision making for their health and wellness.
    • Adheres to ethical, legal/regulatory and accreditation standards, licensing guidelines
    • Other duties as assigned

    Required Qualifications

    • Licensed Nurse Practitioner in NY and CT, preferably with a concentration in geriatrics and is comfortable with conversations around Advanced Care Planning
    • Minimum of two years clinical experience
    • Astute triage skills required
    • Complex Care Management experience desired
    • Transitional coaching experience a plus
    • Home care or SNF experience highly desirable
    • Knowledge of Medicaid, Medicare, and government/CMS regulations preferred
    • Self-directed worker
    • Clinical competence in understanding SNF care and care coordination
    • Bilingual preferred
    • Must have computer skills and technical aptitude
    • Able to travel to several SNF within region for onsite case management
    • Ability to visit patients in their homes
