Position Summary

Westmed Medical Group is seeking an experienced Nurse Practitioner for Care Transition.  This role will be based out of the Rye, New York office location.  The Nurse Practitioner will be assessing, diagnosing and treating the patient in their home and responsible for the transition of care from the hospital.

 

 

 

Essential Job Functions & Responsibilities

  • Collaboration and communication with patient's primary care provider, specialist and discharging hospital
  • Referrals to specialists
  • Referrals to community resources including home care, durable medical equipment & supplies and end-of-life and palliative care options.
  • Ordering labs, tests and medications
  • Medication reconciliation & adherence
  • Prescription writing, orders, refills
  • Patient & family education and support
  • Discharge summary and transition back to the WESTMED primary care physician

Required Qualifications

  • Current licensure in New York and Connecticut as a Nurse Practitioner, preferably with concentration in geriatrics
  • Work experience as a Nurse Practitioner
  • Experience in home care strongly preferred
  • Ability to work independently with minimal supervision
  • Highly motivated, conscientious, and personable
  • Strong communication skills
