Position Summary

Westmed Medical Group has a Nurse Practitioner position available in the Palliative Care department in Westchester County, NY.

 

The Psychiatric-Mental Health Nurse Practitioner is an integral member of the Palliative Care team, participating in a collaborative effort to provide consultation, interdisciplinary service to patients with chronic and/or life threatening conditions for supportive care and symptom management.

Essential Job Functions & Responsibilities

Responsibilities (including, but not limited to):

  • Provide consultation in palliative care, symptom management and supportive care for the patient to meet the general medical needs of the patient to the extent that these are not met by the attending physician.
  • Facilitate clarification of patient and family goals of care.
  • Facilitate access to appropriate support models of care.
  • Provide second opinion regarding need for Hospice care with official certification provided by Hospice Medical Director.
  • Education of physicians, clinical providers and staff.
  • Provide community education.
  • Assist in the development of standards of care.
  • Proactive leadership within healthcare system, participate in work teams, develop and implement care pathways that support complex patient care throughout the system
  • Oversee day-to-day department operational and clinical activities. Plan, direct and coordinate daily activities of a department/group/team.
  • Supervise staff and give technical direction, delegate staff responsibilities.
  • Monitor budget and utilize operational resources, participate in program development.
  • Assure services are in compliance with professional standards, state and federal regulatory requirements.
  • Hire and review non-management employees.

 

Required Qualifications

Education

  • Current or eligible NY licensure as Psychiatric-Mental Health Nurse Practitioner
  • APRN, ANP, ACHPN
  • BLS Certification- Required
  • Minimum 3 years of nursing /NP experience; in palliative care and oncology

Social skills

 

  • Ability to communicate effectively with patients, insurance companies, and co-workers
  • Strong communication skills with MDs on a continuously basis in regards to patient care

Computer skills

 

  • Knowledge of EMR, MS Word/Excel, Palliative Medicine billing codes
  • Knowledge of Budget preparation spreadsheets
